This service provides individuals and/or group counseling, behavioral interventions, diagnostic evaluations or consultations related to the individual’s developmental disability.

This service provides a comprehensive employment and support service. This program focuses on engaging the participant in identifying their career goals and direction by developing a plan according to the individual’s skills and interests to be used to guide the individual employment support.

This service provides direct support and assistance for participants with or without the caregiver present, in and out of the participant’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. Their social skills, and basic self-care and activities of daily living are outlined.

This service is provided outside of a participant’s home to support their educational and/or recreational activities intended to enhance the participant’s social and developmental skills.

This service is to provide direct support and assistance to participants with or without the caregiver present, in or out of the participant’s residence to achieve and/or maintain the outcomes of increased independence, productivity to enhance the family functioning, and inclusion in the community. This includes assistance with community-based activities and assistance to individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.

This service is provided to participants unable to care for themselves that are furnished on a short-term basis because of the absence or need for relief of those individuals who normally provide care for the participant. This service may be delivered in multiple periods of duration and may be provided in the participant’s home, a DHS licensed group home, or another community-based setting approved by DHS.

This service is provided to assists the participant (or the participant’s family or representative, as appropriate) in arranging for, directing and managing services. We serve as the agent of the participant or family, the service is available to assist in identifying immediate and long-term needs, developing options to meet those needs and accessing identified supports and services. Securing resources include locating and securing a place to live (apartment, condo, etc.) and developing, expanding, and facilitating a circle of support.

This service is to enable participants to gain access to services, activities and resources in addition to medical transportation services. It can also be provided by Medicaid/DDD approved transportation providers; generic transportation services/vendors used by the public.

This service is provided to assist the participant to obtain and maintain employment in competitive or a customized work setting in the general workforce. This may be delivered for an intensive period upon the participant’s initial employment in a face-to-face setting, one-on-one, or by phone with the participant and/or their employer. The participants are successfully developing their skills and training to enable integration into the workforce and within the community with training and systematic instruction, job coaching, job placement, job development, negotiation with prospective employers, job analysis, benefit support, travel training, and other workplace support services. This service may also be delivered as a group setting for two to eight workers with disabilities. This may include mobile crews and other business-based work groups employing small groups of workers with disabilities in employment in the community.

This service allows up to three unrelated members to live in a trained caregiver’s home. Our trained support staff provides support and health services for these members.

This service provides teaching to a member and/or caregiver either one-to-one or in a group setting a variety of skills needed for independent living, like coping skills to help the member in dealing with a disability. Coping skills for the caretake to deal with supporting someone with long-term care needs. Skills to deal with providers and attendants.

This service benefits those given to a member who is moving from an institution to their own home. It covers one time moving costs.

This service helps the member to keep the home clean, sanitary, and safe, such as cleaning appliances, cleaning and securing rugs and carpets, washing walls and windows, scrubbing floors, cleaning attics and basements to remove fire and health hazards, clearing walkways of ice, snow, leaves, trimming overhanging tree branches, replacing fuses, light bulbs, electric plugs, frayed cords, replacing door locks, window catches, replacing faucets washers, installing safety equipment, seasonal changes of screens and storm windows, weather stripping around doors, and caulking windows. Occasional heavy household maintenance tasks to ensure the members’ safety.

This service includes a personal care assistant to manage the household duties and health-related tasks. The PCAs are available to accommodate long-term chronic or maintenance health care. This also includes services for members with mental illnesses.

This service is provided on a short-term basis to members who can’t care for themselves. Services given to members who can't care for themselves are given on a short term basis because an unpaid, informal caregiver is unavailable or needs help.

This service serves an individual with household chores or errands like doing laundry, making meals, light housekeeping or grocery shopping. Designed to help MLTSS members with their instrumental activities of daily living needs. HBSC is available to members whose activities of daily living needs are given by non-paid caregivers such as family members or as a wrap around service to non-Medicaid programs.

This service supports beneficiaries in obtaining housing, including but not limited to developing an individualized housing support plan, assisting with navigating the complexities of the housing application process through the progression of prospective tenant to tenant as well as assisting with the housing search, identifying, coordinating, and securing resources to assist with housing cost and other expenses.

This service supports beneficiaries by achieving their goal of maintaining safe and stable tenancy, including but not limited to: developing or revising an individualized housing support plan, assisting with the housing recertification processes, including lease renewals and housing subsidy renewals, educating and training the beneficiary on the role, rights and responsibilities of the tenant and landlord, supporting beneficiary in development of independent living and tenancy skills, identifying and helping secure benefits or supports to help pay for rent and utilities, and providing assistance in addressing circumstances or behaviors that may jeopardize housing such as late payment, lease violation, maintenance issues, disputes with landlords or neighbors, or other identified issues.
